Asphyxial
Deaths

a- without, sphyxis pulse. The Greek name promises a stopped pulse, yet the heart usually beats on for a while after breathing fails. So the word is a misnomer, and today it simply means death from want of oxygen.

breathing, then struggle, then silence

Six ideas to carry out of this chapter

Every death here ends in the same place: cells that can’t get or use oxygen. What differs is where the chain broke.
The “classic signs of asphyxia” are real findings, but not one of them is specific. The neck, the mouth, the chest and the scene make the diagnosis.
Hanging leaves an oblique mark rising to the knot. Strangulation leaves a mark that runs round. Learn why, and the rest of the comparison follows.
Only 4 to 5 kg on a ligature closes the carotid arteries, so a person can die of hanging with the feet on the floor.
In drowning, look for what only a living person could have done: breathe in water, froth, grip the weeds, carry diatoms to the marrow.
Your job ends with the findings and a reasoned opinion. Suicide, homicide or accident is finally the court’s word.

This chapter deals with hanging and suicide. If it touches something personal, it’s fine to pause. Tele-MANAS, India’s free mental health line, answers on 14416.

Study notes

Each point gives the term first, then one plain sentence on what it means. The source sits beside it so you can go back to the book.

Post-mortem findings

The crux of the chapter. What the autopsy shows in each kind of asphyxial death, which findings prove the person was alive, and the artefacts that fool people.

Read the body in order

Artefacts that fool people

These can appear in any neck. Knowing them stops a natural death being read as a killing.

Neck lab

Most of the hanging and strangulation comparison follows from one fact: in hanging, the body’s weight pulls the ligature up towards a fixed knot. Switch between the two and watch where the mark goes.

Sort the finding

Each card is a finding at autopsy. Decide which death it points to more strongly. Strangulation here means ligature strangulation unless the card says otherwise.

Memory tricks

These are memory devices made for this page, not statements from the books. Each one carries the fact it helps you hold, with its source.

Flashcards

Say the answer out loud before you turn the card. Star the ones that slip away, then study only the starred pile.

Test yourself

Pick an answer and read the reason, whether you were right or not. The reason is where the learning sits. Keys 1 to 4 work too.

Case files

Short scenes, the kind you’ll meet in a viva or at your first inquest. Read the findings, then decide.

People and fun facts

Half the eponyms in this chapter are people. Knowing who they were makes the names stick.
